Pacific Assistance Dogs Society (PADS)
PADS breeds, raises, trains and supports certified assistance dogs, partnering them with people living with disabilities, and with community care professionals. All contributions go toward allowing a person living with a disability to live independently with a PADS dog by their side. Form has specifically sponsored a male Labrador x Golden Retriever named Lore (pictured above), born on October 16, 2018. Lore is being raised and trained to be an assistance dog for someone with a physical disability or who is deaf or hard-of-hearing. Canuck Place Children’s Hospice
Canuck Place Children’s Hospice is a place of comfort and compassion where children can receive expert clinical care while experiencing the simple joys of being a child with their families at their side savouring their time together. Canuck Place Children’s Hospice first opened its doors in November 1995, to become North America’s first free-standing children’s hospice. They deliver medical respite, pain and symptom management, art, music, and recreation therapy, end-of-life care, and grief, loss, and bereavement counselling – all at no cost to families. Form supports and attends the annual Gift of Time Gala with all proceeds going to further the mission of Canuck Place Children’s Hospice.
Family Services North Shore
Form is happy to support the Family Services of the North Shore by attending the annual Winter Family Ball. The organization annually serves close to 7,400 individuals and families through a broad range of social services to children, youth, adults, couples, and families from a diverse array of cultural and economic backgrounds. The money raised at the Winter Family Ball goes toward supporting individuals who are struggling and need help in the form of counselling, family support, and education. The evening is also an opportunity to create conversations with children from the organization about empathy for others and how giving back can not only change the world they live in, but their own lives too.
Lions Gate Hospital Foundation
Form serves on the Young Professionals Committee for the Lions Gate Hospital’s annual Nip/Tuck Lawn Bowling Classic. They have been involved with the event since its inception in 2013. The event raises nearly $50,000 annually to support upgrades and new hospital developments. In past years, the funds raised at the event have contributed to building a new Youth Mental Health Unit, redesigning the Ambulatory Care and Outpatient area, as well as building a new medical and surgical centre. Form is also proud to be a Venue Sponsor for the event.
